Maternity Leave Request Letter

Dear Ms. Aarons

I am writing to inform you that I am currently 7 months pregnant with my first child, who is due around the end of July 2010. I would like to request that maternity leave be granted to me from mid July 2010. I have attached the required maternity leave request forms for your perusal.

According to my leave records with the HR department, I am still entitled to 15 days of annual leave which I would like to add on to my maternity leave entitlement.

Please confirm this request and revert on the exact date when I should report back for work after the delivery of my baby. Also, kindly inform me of anything else which I would need to clear before my leave commences.
