Subject: Letter for appeal on insurance matters.

Dear Sir/Madam;

I the undersigned prospective policy holder of your branch; in relation to the subject would kindly request to look in to refund case of policy number L-4657860 dated & issued on 4th Feb’ 2000. Having inquired more than a couple of times at your Powai branch regarding the same issue; but the response was not at all convincing.

Having made regular payment installments in the Long Life Line Policy & also without any defaults; Sir, this appeal has been drafted to request you to look into the matter since its pending from the last 3 months.

Lastly having lapsed since the last 3 months it is my sincere appeal to refund the sum assured amount without consuming more time in completing the paper formalities. Enclosing herewith the necessary policy documents.
